1	Drug_Class	Drug class or group of drug classes if a co-occurance; Drug class is derived using RXNorm drug classifications. 
2	Drug	Name of the drug's active ingredient
3	Cooccurrence_flg	A co-occurance is a dispensation of one drug type within the range of fill date + days supply of another drug type. Co-occurances are defined for opioids, benzodiazepines and skeletal muscle relaxants of concern (cyclobenzaprine & carisoprodol). A value of 0 indicates that the record is not a co-occurance and a value of 1 is a co-occurance.
4	Dispensation_Prescription	Denoting whether the data is related to a prescription or dispensation. Prescription reflects the order written or transmitted by a licensed prescriber. Dispensation reflects the fulfillment of the prescription, when the medication is provided to the patient. 
5	County_Type	Denotes whether the county is related to the Practitioner, Pharmacy, or Patient
6	County	County that the Aggregated_count is based on. 'All' reflects counts for entire state. 
7	Count_Category	Primary category that the count is aggregated to
8	Count_Subcategory	Subcategory (the more granular category) that the Aggregated_count is based on
9	Year	Year of prescription
10	Aggregated_Value	Value of aggregated count or rate per 1,000 county residents. *If Subcategory is an Indiana county then this count is per 1k county residents
11	Total_or_per_1k	Denotes whether the aggregated count is total for the state or per 1000 county residents
